When is this needed?
- Late registration: If you missed the 21-day window, you’ll need this affidavit when submitting documents after the due period, along with hospital or other evidence.
- Home births or missing records: It serves as a sworn statement for cases lacking official proof.
- Stamp paper & notarization:
- Use ₹20 or above stamp paper.
- Affidavit must be sworn and signed in front of a Magistrate or Notary (self‑declaration is not accepted).
- Where to file:
- Submit the affidavit along with your application at the local Municipal Birth & Death Registrar or CSC/Nagar Nigam office.
Tips for Filling Out
- Ensure complete and accurate address details, including ward/sector and post office.
- Be precise with the date and place of birth—if exact hospital/area isn’t known, indicate approximate location.
- Reason for delay should be brief but honest (e.g. home birth, unaware of deadline, no hospital documentation).
- Keep a photocopy of the affidavit and any attached documents.
